If you have spoken with a FOIP specialist and would like to make a formal request, please access the forms below.

• Access to personal and general information: fill out the online request form here.
• To change your personal information, fill out the FOIP Request to Correct Personal Information Form.
• If you prefer to make your request in person, contact legislativeservices@highriver.ca for a copy of the FOIP Request to Access Information Form.

Submit the completed request forms and the applicable fees by email to legislativeservices@highriver.ca or Legislative & Advisory Services 309B Macleod Trail SW High River, Alberta, T1V 1Z5.

No initial fee is required for access to personal information. However, if you request access to general information, an initial fee of $25 is required before the FOIP Coordinator can begin the process.

There may be additional charges associated with more extensive and expedited requests. For all additional inquiries, refer to the fees outlined in the current rate bylaw.

If you have concerns with the Town's decision or response to your FOIP request, you can submit the Request for Review/Complaint Form to the Office of the Information and Privacy Commissioner (the 'OIPC').
